The ACS module of Cidade Saudável is a system that can be purchased by municipalities, installed for monitoring by the Health Department, and used by Community Health Agents (ACS).

This application allows the replacement of paper forms, used by Community Health Agents, with electronic forms filled out on tablets.

Cidade Saudável ACS works online and offline, among other features, the following stand out:

● Carrying out Individual Registration, Household Registration, Home Visit, among other frequently used forms;
● Sending registered forms when connecting to the internet;
● Obtaining coordinates for patient geolocation;
● Statistical data analysis.
