About this app

A study app for Japan's Certified Heart Failure Educator exam (心不全療養指導士認定試験). This certification is awarded by the Japanese Circulation Society to healthcare professionals who support patients and families across the whole course of heart failure - prevention, acute care, chronic management and palliative care - as part of a multidisciplinary team. All 418 practice questions come with a clear explanation.

■ Subjects Covered (418 questions across 10 areas)

• Role of the educator, concept and epidemiology of heart failure (36) - background to the certification, definitions, stage classification
• Fundamentals of patient education (41) - disease management programmes, behaviour change, adherence, self-care assessment
• Prevention (25) - intervention at stages A and B, risk factor management, health literacy
• Cardiac structure, function and pathophysiology (41) - anatomy and valves, systole and diastole, neurohormonal factors, HFrEF and HFpEF
• Physical findings, tests and diagnosis (49) - diagnostic process, congestion, echocardiography, BNP and NT-proBNP, ECG and chest X-ray
• Pharmacological treatment (60) - GDMT, ACE inhibitors and ARBs, ARNI, beta blockers, MRA, SGLT2 inhibitors, diuretics
• Non-pharmacological treatment, comorbidities and pregnancy (46) - ICD and CRT, mechanical support and transplantation, renal disease and anaemia
• Practical patient education (64) - salt and fluid management, nutrition, cardiac rehabilitation, medication support, lifestyle, psychological care
• Education by stage and in special situations (25) - older patients and frailty, acute decompensation, disasters and infectious outbreaks
• Palliative care, teamwork, home care, social resources and records (31) - shared decision making, ACP, discharge planning, information sharing

The exam is held every December in multiple-choice format, and its scope follows the official guidebook published by the Japanese Circulation Society. This app is organised into the same ten areas as the guidebook, so you can drill each section in the order you studied it. Exam dates, format and passing criteria can change from year to year, so please check the official site for the latest information before applying.

Pathophysiology and pharmacological treatment carry the most weight, and the mechanisms and titration of the drugs that make up GDMT come up again and again. This app includes 60 questions on drug therapy alone and 90 on pathophysiology, tests and diagnosis, and records your accuracy per area automatically so you can work through your weakest sections first.

Note that eligibility is restricted: candidates must be members of the Japanese Circulation Society, hold a relevant national licence (nurse, public health nurse, pharmacist, registered dietitian, physical or occupational therapist, certified public psychologist, clinical engineer, dental hygienist, certified social worker and others), complete the required e-learning, and submit case reports. See the official site for the full requirements.
