The Breakthrough Digital Health app connects you with your wellness plan from Breakthrough Health Centers. Receive daily meal planning, exercise guidance, lifestyle modification support and motivation.
As a Breakthrough Health Centers patient, you will receive the specific content of your wellness plan in the app. Track your progress including calories, activities, body metrics, steps, water, and sleep in an interface shared with your provider. Includes an integration with Innotech, Fitbit and other connected health devices. Communicate with Breakthrough Health Centers in realtime through personal journals and direct private messages.
