HUBCHART app is a free HIPAA compliant, Patient-Centered Collaborative Care app. It is the best tool to manage thousands of Patient Care Groups. The Care Team can securely share vital information with each other anytime, anywhere. This Team Care approach helps facilitate and expedite care delivery. The “Patient-named” Collaborative Chat Groups can function like an ad-hoc Health Record where all chats, files, images, videos, and link can be shared within the secured group and archived for future reference. These private group chats are managed by a group owner and administrators, as well as other participants of the care team permitted by the group owner.
