Achieving optimal mental health requires that you work with different health practitioners. Often times, care is disjointed, and everyone is not on the same page, affecting your journey towards your wellness goals.

With Care Plan 365, the consumer (using this APP) and their GP create a mental health care plan that their psychologist and psychiatrist can contribute to, all on the same platform, leading to shared care funded by Medicare Australia.

Symptom Checker - Complete an online assessment and get insights into your mental health

Stressors - identify areas of life that cause you stress

Goal setting - set your long-term and short-term goals for your mental health to be shared immediately with health practitioners

Connect on the same platform
attend appointments with your GP to complete and regularly review your mental health care plan
use your Medicare sponsored care plan to access a psychologist
visit with a psychiatrist using referral from your GP

Tasks & Track
view and complete tasks agreed with your mental health team
keep tabs on your symptoms and your goals on your mental health journey
