Intercept is designed to help patients who had a recent cardiac event, self-manage their health. With access to a range of features, the app supports rehabilitation and provides tools to track wellbeing, build daily schedules, and add goals. Intercept facilitates self-management and monitoring outcomes: healthier lifestyles, improved risk factor control and adherence with cardioprotective medications.

The aim is to empower patients to take control of their recovery by improving their understanding of their health and increasing their motivation to improve their lifestyle.

To help ensure the app meets with user needs it has been designed and developed by health care professionals in collaboration with patients living with heart disease.
