The app helps track health information from compatible external devices provided as part of your healthcare services, enabling providers to stay connected with patients for education and self-care support.

Requirements: This app requires compatible external health monitoring devices (e.g., Bluetooth-enabled blood pressure monitors, weight scales, or pulse oximeters). These devices are supplied to patients through their healthcare provider and are necessary for full functionality.

Compatibility: Works with Android devices that support Bluetooth Low Energy (BLE) and standard device sensors.

Your personal health information (PHI) is protected with secure bi-directional communication with your healthcare team and is HIPAA compliant.
