With EmmaCare you will be able to reduce your gaps in care and provide your Care Manager with essential and real-time information about your health. Your interactions with your Care Manager will become much more meaningful and will focus on your specific healthcare problems.

With EmmaCare your Care Manager will:

- Help you manage your specific illness
- Periodically check-in on your health
- Schedule your weekly/monthly appointments
- Provide you with logistical help
- Help you manage your medication
