Care Elevate is a digital care management app that connects you with your care team for one-on-one, personalized support – right from your phone.

You can send private messages to your care team anytime – it’s as easy as texting, but more secure. Whether you have questions, need help managing your health, or want guidance between doctor visits, your care team is there to support you and help you feel more confident and in control of your health.

With Care Elevate, you can:
• Message your care team securely, anytime
• Set reminders for medications and appointments
• Track steps and monitor progress over time
• Access personalized health tips and resources tailored to your needs

Care Elevate helps you stay on top of your health between visits, so you can focus on what matters most.

Your privacy is our priority – Care Elevate is fully HIPAA compliant, using secure technology to protect your personal health information. The app is provided at no extra cost to eligible members as part of your plan benefits. Getting started is easy: just sign up with the access code provided by your health plan and have your member ID (from your insurance card) handy.

Who can use this app: Care Elevate is available by invitation only. If you received an invitation code from your health insurance plan, you’re eligible to use the app.

What’s new

Android members can now sync blood glucose readings directly from Google Health Connect into assigned surveys, no more manual entry, supporting more accurate biometric tracking with less effort.
