NavigateHER is a medical device–enabled mobile application that provides clinical decision support and care management by integrating real-time vital sign data, social determinants of health (SDOH), and mental health screening tools.
The app enables the healthcare team to make informed, timely clinical decisions using validated remote patient-monitoring data, including an FDA-cleared blood-pressure cuff validated for use in pregnant, pre-eclamptic, and postpartum patients, as well as an FSA-eligible, FDA-cleared scale. These data points are combined with patient-reported SDOH and mental health questionnaires to support risk stratification, early intervention, and ongoing care management.
NavigateHER also supports mental and behavioral health by administering the Edinburgh Postnatal Depression Scale (EPDS). Based on screening results, the healthcare care team can identify patients who may need additional support and facilitate referrals or direct patients to appropriate mental health services as clinically indicated.
In addition, the app serves as a medical reference and education tool through its built-in education library, providing patients with evidence-based educational resources tailored to pregnancy and postpartum care, supporting patient engagement, self-management, and adherence to care plans.
