Sage Wound Consultant is structured to provide even the novice wound clinician with the tools to document like an expert while also providing the expert with an easier means for documentation. The app will guide the clinician through a thorough assessment of the wound/wounds to ensure a holistic approach to wound care.

This allows automation of your wound care documentation in a faster, efficient and less costly manner to improve compliance and patient outcomes. The app has been designed to minimize time to document by utilizing checked bullets while also allowing for free typing if needed to customize the report. The latest recommended guidelines are provided throughout the assessment. A concise final report can be emailed to multiple recipients if needed for medical records.

A hard copy report can be printed from the email. The final report provides a thorough documentation of the assessment which includes not only the description of the wound but medications delaying or supporting wound healing, risk factors contributing to wound etiology, past medical history, nutritional status, recommended referrals, treatment orders or recommendations, and a progress note from the visit.

Updates to the record can be made from the final pdf screen without going into the entire app to minimize the time needed to document each encounter. Follow up assessments can then be easily updated to improve productivity. You can review our privacy policies here.
