A unified patient roster with rich, filterable views. Search, sort, and segment your panel by program, status, provider, health coach, or any custom criteria. View detailed patient records including demographics, contacts, addresses, insurance, providers, devices, and care history.

Vitals & Remote Patient Monitoring
Track blood pressure, blood glucose, pulse oximetry, weight, and pulse readings from connected external monitoring devices. Visualize trends on time-series charts, set alert thresholds, and generate signed PDF vitals logs for the chart. Note: Vitals capture requires compatible external hardware devices such as blood pressure monitors, glucometers, pulse
oximeters, or weight scales. This app does not capture vitals independently without these devices.

Encounter Documentation
Create and review structured encounter notes for Chronic Care Management
(CCM), Basic CCM, Remote Patient Monitoring (RPM), Interdisciplinary Care
Team (ICT) meetings, Patient Assessments, Care Plans, Transitional Care
Management (TCM/TOC), Patient Activities, Medication Reviews, and 24/7
Coverage encounters.

Care Plans
Build and update problem-oriented care plans with goals, interventions,
and follow-up notes. Track progress over time and surface care plan
elements directly inside encounter workflows.

TCM & ADT Alerts
Receive admission, discharge, and transfer notifications, manage
post-discharge follow-up, schedule and document Transitions of Care
assessments, and close the loop on every discharge event.

Operations & RPM Dashboards
Real-time dashboards for operations leaders covering CCM, RPM, ICT,
assessments, follow-up adherence, device lifecycle, alert volume, and
program compliance.

Patient Notes & Messaging
Capture rich-text patient notes with full version history. Send and
review SMS messages with patients directly inside the patient record.

Manage and share patient-education materials and clinical references via
secure cloud document storage.

Save personalized grid configurations, share views across teams, and use
full-text search across the entire patient panel.

Reporting & PDF Generation
Generate clinician-attested PDFs for vitals, care plans, assessments,
and encounters ready for the chart, the payer, or a compliance review.

Data Privacy
Clinicus may access health data to support clinical documentation and
care workflows. Any health data accessed is used solely to support care
delivery by authorized clinical staff, is never sold to third parties,
and is never used for advertising or marketing purposes. All data is

Who This Is For
Clinicus is a professional clinical tool intended for use by authorized
healthcare staff at organizations that have licensed the platform. It is
not a consumer health app and not a patient-facing self-service tool.
Access requires a valid organization account provisioned by your
administrator. Patients who have questions about their care should
contact their care team directly.
