About this app

SeamlessMD is a digital care companion prescribed by your hospital or healthcare provider. Your care team enrolls you in a program designed for your specific surgery or health condition — such as surgery preparation and recovery, heart failure, high blood pressure, diabetes, COPD, or other chronic condition management.

Note: You need an enrollment code or invitation from your healthcare provider to use this app. It is not available for self sign-up.

TRACK YOUR HEALTH — AUTOMATICALLY Depending on the program your care team prescribes, you can record health measurements in the app, or sync them automatically from your connected devices and wearables through Health Connect: • Blood pressure — for high blood pressure, heart failure, and cardiac surgery programs • Blood glucose — for diabetes management and surgical glucose monitoring programs • Weight — for heart failure programs, where daily weight changes help your care team detect fluid retention early • Heart rate and oxygen saturation — for cardiac, respiratory, and post-surgery monitoring programs • Steps — to track your activity and mobility goals during recovery

Some programs use external health devices such as a blood pressure cuff, glucose monitor, pulse oximeter, or weight scale. Your care team will let you know what your program needs.

YOUR CARE TEAM IS WATCHING OUT FOR YOU The measurements and health check answers you share are monitored by your hospital care team. If a reading falls outside the range your care team set for you, they are alerted and can reach out — helping catch problems early, often before they become emergencies.

YOUR PRIVACY Your health data is only shared with your healthcare team. It is encrypted in transit, never sold, and never used for advertising. You choose which data to sync, and you can disconnect Health Connect at any time.

This app is confusing and also too repetitive. I'm constantly needing to do the same tracking process over and over, it keeps asking me if I've completed labs and cardiac and I've already noted that it has been completed. It asks about my weight every once in a while. It would have been more effective if I could go in daily and put in my weight and food, but it's not set up that way. Also, there seems to be a disconnect with my organization and the app. please fix the app and make it better.

This app is absolutely amazing. Connecting you with a health care service in real time and knowing a nurse is on the other side able to review your submissions is exactly the peace of mind we needed in our situation. I just wish more hospitals and doctors offices had use of this! We are two hours away from London so the "local services" offered through the app don't work for us however, it did spur us to look into what is offered locally to our area.

the app itself does what it says, and would be 5 stars if not fot the very frequent crashes. as well the bottom app navigation buttons overlapped the phone's own "Home", "Menu" and "Back" buttons.
