LynCare is a care coordination app built for self-directed Medicaid families and the caregivers who support them.
If you or someone you love receives self-directed care, you know how much falls on family members and trusted caregivers — managing medications, coordinating shifts, keeping track of daily activities, and making sure nothing gets lost between caregivers. LynCare brings all of that into one place, designed by someone who understands what self-directed care actually looks like day-to-day.

• Medication tracking — Track routine medications (eMAR) and as-needed (PRN) medications with clear schedules and administration records. Reduce the risk of missed or doubled doses.
• Daily notes and handoff — Caregivers document the day's events, observations, and concerns. Notes are preserved as part of the participant's care history so nothing gets lost between shifts.
• Shift management — Set up shift schedules, clock in and out, and see who's on duty at any time.
• Tasks and ADLs — Manage daily activities of living and periodic tasks. Assign tasks to specific shifts or rotate through your team.
• Work communication — Send messages and broadcasts to your care team without relying on group texts.
• Calendar coordination — Schedule appointments and care events. Optional Google Calendar integration keeps everything in sync.
• Role-based access — Different roles for participants, advocates, caregivers, admins, and care managers ensure everyone sees what they need to.
