提供安全的運輸服務為軌道營運業者的首要目標之一，目前風險管理的技術已被廣泛應用在軌道運輸系統來提升安全水準，其中風險辨識為執行風險管理的首要工作，也是後續風險分析及風險評量的依據，營運單位可經由風險辨識的過程找出系統潛在的安全危害藉以研擬因應策略。 本研究應用「類似系統比較」、「臺鐵歷史事故回顧」、「專家腦力激盪」等相關方法，以臺鐵為例研擬安全危害清單之雛形，並辨識出113項安全危害，主要區分移動、非移動、列車與天然危害四大類，該成果可協助臺鐵進行安全風險管理，並作為後續風險分析與風險評量的依據。 本研究選定臺鐵兩項平交道危害，利用失誤樹與事件樹分析法進行深入分析，除探討所有可能導致危害發生的失效事件外，影響危害後果嚴重度的關鍵事件亦是分析重點，並根據分析結果建議相關改善方法，期提昇臺鐵平交道安全。
