HealthJay is a care engagement app offering person-centered access to health care recipients. Clinical care, chronic care, and social determinants of health are all delivered including walkie-talkie video chat with care providers. Users can manage care events in this ultra simple-to-use app using a whole-health approach. This gateway app takes care recipients and care providers through care journeys with several transformative digital breakthroughs:

• All communications are automatically translated into the users selected language with both text and high quality speech synthesis

• The user chooses their preferred language from a selection of 25 languages
