• Private Messaging: communicate privately with your Proactive MD provider and care team any time of the day or night with secure messaging.
• Test Results: see your lab results and health history instantly without scheduling an appointment.
• Access Medical Records: easily access and view your electronic medical record online.
• App-to-App Integration: integrate with Apple HealthKit to retrieve daily exercise logs, sleep patterns and health data

To use the Proactive MD Portal, sign up at your Health Center or sign up at home by contacting your Health Center to confirm your information and request an email invitation.

Proactive MD Portal is a healthcare services and patient management app designed exclusively for patients of Proactive MD Portal.

The app integrates with Android Health Connect to read user-authorized health and fitness data, such as activity, sleep, and vital metrics, and display it inside the app to support patient awareness and continuity of care.

Health Connect access is read-only, opt-in, and used only to display health information within the app. The app does not provide medical diagnosis or treatment and does not sell or use health data for advertising purposes.

Users can revoke Health Connect access at any time through their device settings. Access to the app requires authorization from Proactive MD Portal.
