About this app

Encompass Health inpatient rehabilitation patients and their designated caregivers have the option to use the MyEncompass Health Caregiver application to follow along with a patient’s rehabilitation program. The app shows a patient’s real-time progress toward their goals and an overview of their care plan in a secure manner.

Information in the app is regularly updated by our care teams based on the individual care plan for the patient.

TRACK
- Follow the patient’s stay at our hospital as they make progress toward established goals.
o Track goals established by the patient and the hospital's interdisciplinary care team, including mobility, self-care, cognition and behavior, and daily living skills such as meal preparation and medication management.
o See anticipated discharge date.
o Plan for discharge, with instructions that may include:
 Discharge diagnosis
 Instructions for care from nursing and therapy
 Follow-up appointments
 Medications
 Other pertinent information for care at home

SHARE
- Invite others to follow along as the patient progresses in their rehabilitation stay.
- Allow the patient’s legal representative to share their information with others, through granting access to the application on their behalf.

The patient and those who are granted access to the app will have access up to 14 days after discharge.

Ratings and reviews

I am unable to sign up though I entered my husband's information multiple ways. Not sure if this is case-sensitive but no version of his name worked. I tried his legal first name as well as the name he is called. Every time I tried I received an error message indicating "No current patien found with that name date of birth, zip code" which is strange considering I am sittinnext to his bed on their building. So frustrating!

Very frustrating. Tried downloading in hospital but said no internet connection. Tried downloading at home With good WiFi connection. Able to install the app BUT when signing up it kept coming up with red bar saying no internet connection. Tried several times. Very time consuming and frustrating.

Limited value and negligible assist for family- care team communications. App opens with reminder to share thoughts with care team but app provides no communication link. In a care category, if you open a specific goal, there is no way scroll to next goal in that category; you have to back out and find next goal to check it, etc etc.

What's new

Patients can now pre-authorize friends and family to track their progress. These are "designated users" and can sign up through a simple confirmation process.
