You are connected to your healthcare provider via the DEARhealth app. In the app you can see your care pathway, questionnaires, your scheduled appointments and your health assessment score. This will give insight into your disease progression over time. You receive education about your condition and tips to improve your health.

You can provide your provider updates in case something in your health situation has changed. Your provider can take action if needed. If your health progression goes better than expected, it could be that some check-ups are not necessary.
