WELL is a patient and provider facing, app-based platform designed to support both sides of care as one coherent system. It uses AI to help patients share their medical story before a visit and helps clinicians start each encounter with a clearer, more structured understanding. WELL also gives clinicians advisory decision-support tools during visits - the clinician makes all clinical decisions.

Your personal health record, owned and controlled by you. Add conditions, medications, and medical history to build a complete picture of your health over time.

AI Assistant Chat

Ask general health questions and get educational information informed by your own health profile stored in WELL, rather than generic responses. Answers are for information only and are not medical advice or a diagnosis - discuss anything health-related with your provider.

What’s new

• Providers can now see a patient's full Medical Briefcase: active conditions, past medical history, surgeries, medications, lab and imaging results, allergies and immunizations from connected health records
• Condition summaries on the provider side are now written for clinicians, with dates, sources and a clear note of what the records don't show
• Medications the records link to a condition are now shown to providers, with the reason recorded
